There are two forms required for Trek:

1. Registration Form (digital) - see below. Used for getting a count and dividing stake youth into trek families.
2. Permission Slip\Medical Release\Photo Release Form (paper) - this must be printed, filled out, and returned to a member of your ward bishopric or youth leaders who will then give them to Sister Triessa Hepworth, Trek Communications Chair.
